Instructions for Applying for Renewal of your KYCP Certification
Following are complete instructions for successful completion and submission of all the required documentation to renew your certification with Kentucky Youth Care Professionals. To download the required forms, please click on the designated link in the description below. To view or print the downloaded documents, you must have Adobe Acrobat Reader (version 5.0 or later) installed on your computer. Acrobat Reader is free and can be downloaded from the Adobe website by clicking the Acrobat icon below.
Please complete and submit the following:
(In order for your application to be considered, all of the following documents and fee must be included)
1. Completed Re-Application form, including the Completed KYCP Training Record Document (submit a copy of your training records from your agency only if your letter is stamped “Audit”)
2. Criminal background check and Child Abuse and Neglect registry check – completed between August 2004 and October 2006
3. Fee – check or money order payable to KYCP, Inc.
Level I - $20.00
Level II - $30.00
Level III - $40.00
Level IV - $50.00
Please Note:
You will need to request a copy of your criminal background check, child abuse and neglect registry check and official training record (if letter is stamped “audit”) from your employer. Check with your Human Resources department.

The KYCP, Inc. Certification Review Committee will review all of the required documents and contact applicants regarding the status of your renewal.
